El daño cerebral en el traumatismo craneoencefálico severo: estudio de un caso de intervención neuropsicológica

Abstract: Los traumatismos craneoencefálicos (TCE) severos son un grave problema de salud pública, tanto por sus altas tasas de mortalidad, como por las discapacidades que presentan los pacientes que sobreviven. Las lesiones cerebrales consecuencia de los TCE pueden producir alteraciones físicas, sensoriales, cognitivas, conductuales y emocionales. Estas alteraciones causan un fuerte impacto a nivel personal, familiar, social y laboral. En este trabajo se presenta un caso de un paciente con daño cerebral por TCE severo. Se trata de un paciente ficticio basado en mi experiencia durante la realización de mis prácticas en la Unidad de Daño Cerebral del Hospital Beata María Ana de Madrid. La evaluación del paciente muestra un perfil de alteraciones neuropsicológicas entre moderadas y graves. A nivel cognitivo presenta fatiga, alteraciones atencionales, heminegligencia izquierda, enlentecimiento en la velocidad de procesamiento de la información, alteraciones en la memoria operativa y en el aprendizaje, apraxia constructiva, dificultades en el acceso al léxico y alteraciones en las funciones ejecutivas. A nivel conductual muestra desinhibición, egocentrismo y anosognosia. La intervención neuropsicológica propuesta consiste en 3 sesiones individuales semanales de 45 minutos durante 9 meses. El objetivo principal de la intervención será reducir el impacto de las deficiencias cognitivas y las alteraciones conductuales del paciente que impiden el funcionamiento adecuado en su vida diaria. La intervención incluye psicoeducación, estimulación cognitiva, estrategias compensatorias, psicoterapia e intervención familiar. Tras la intervención se espera una mejora significativa tanto a nivel cognitivo como conductual, y una mayor autonomía y ajuste social en el paciente. ; Traumatic Brain Injury (TBI) is a serious public health problem, both because of its high mortality rates and because of the disabilities presented by patients who survive. TBI brain injuries can cause physical, sensory, cognitive, behavioral and emotional disorders. These alterations cause a strong impact on a personal, family, social and labor level. This paper presents a case of a patient with severe brain damage due to severe TBI. This is a fictional patient based on my experience during the performance of my practices in the Brain Damage Unit of the Beata María Ana Hospital in Madrid. The evaluation of the patient shows a profile of neuropsychological alterations between moderate and severe. At the cognitive level it presents fatigue, attentional alterations, left heminegligence, slowdown in the speed of information processing, alterations in operational memory and learning, constructive apraxia, difficulties in accessing the lexicon and alterations in executive functions. At the behavioral level it shows disinhibition, egocentrism and anosognosia. The proposed neuropsychological intervention consists of 3 weekly 45-minute individual sessions for 9 months. The main objective of the intervention will be to reduce the impact of cognitive deficits and behavioral alterations of the patient that impede proper functioning in his daily life. The intervention includes psychoeducation, cognitive stimulation, compensatory strategies, psychotherapy and family intervention. After the intervention, a significant improvement is expected both cognitively and behaviorally, and greater autonomy and social adjustment in the patient. ; Els traumatismes cranioencefàlics (TCE) severs són un greu problema de salut pública, tant per les seves altes taxes de mortalitat, com per les discapacitats que presenten els pacients que sobreviuen. Les lesions cerebrals conseqüència dels TCE poden produir alteracions físiques, sensorials, cognitives, conductuals i emocionals. Aquestes alteracions causen un fort impacte a nivell personal, familiar, social i laboral. En aquest treball es presenta un cas d'un pacient amb dany cerebral per TCE sever. Es tracta d'un pacient fictici basat en la meva experiència durant la realització de les meves pràctiques en la Unitat de Dany Cerebral de l'Hospital Beata María Ana de Madrid. L'avaluació del pacient mostra un perfil d'alteracions neuropsicològiques entre moderades i greus. A nivell cognitiu presenta fatiga, alteracions atencionales, heminegligencia esquerra, alentiment en la velocitat de processament de la informació, alteracions en la memòria operativa i en l'aprenentatge, apràxia constructiva, dificultats en l'accés al lèxic i alteracions en les funcions executives. A nivell conductual mostra desinhibició, egocentrisme i anosognosia. La intervenció neuropsicològica proposada consisteix en 3 sessions individuals setmanals de 45 minuts durant 9 mesos. L'objectiu principal de la intervenció serà reduir l'impacte de les deficiències cognitives i les alteracions conductuals del pacient que impedeixen el funcionament adequat en la seva vida diària. La intervenció inclou psicoeducación, estimulació cognitiva, estratègies compensatòries, psicoteràpia i intervenció familiar. Després de la intervenció s'espera una millora significativa tant a nivell cognitiu com conductual, i una major autonomia i ajust social en el pacient.
