El arte de la observación y empatía en la especialidad de Psiquiatría. Luces y sombras

Abstract:
A diferencia de otras especialidades de la medicina, la psiquiatría se basa sobre todo en la observación clínica del paciente. Por lo tanto, para identificar la psicopatología es importante no solamente una formación teórica, sino que se deben desarrollar habilidades como la empatía y la observación del lenguaje no verbal. Sin embargo, es un arte que necesita perfeccionarse y entrenarse, sobre todo durante el período de formación especializada. Cuando existe un vínculo afectivo con el paciente y cuando un paciente está bajo tratamiento psicofarmacológico, las expresiones faciales observadas pueden interpretarse de forma errónea. El arte de la observación implica también, saber adaptar el tiempo de la mirada al paciente para evitar que se sienta amenazado. La observación es la base del método científico y la observación objetiva de la conducta es una de las herramientas más valiosas en la psiquiatría. Por tanto, proponemos adaptar la metodología de estudios observacionales previos para sistematizar la identificación de las emociones y expresiones faciales de los distintos trastornos psiquiátricos. De esta manera, mejoraría la calidad de la formación especializada, sobre todo en los profesionales con valores extremos de empatía. ; Unlike other specialties of medicine, psychiatry is based principally on clinical observation of the patient. Therefore, to identify psychopathology, not only theoretical training is important, but also skills such as empathy and observation of non-verbal language must be developed. However, it is an art that needs to be improved and practiced, especially during the specialist medical training. When there is an emotional ties with the patient, and when a patient is under psychopharmacological treatment, the observed facial expressions can be misconstrued. The art of observation implies adapting the gaze time to the patient, to avoid feeling threatened. Observation is the basis of the scientific method, and objective observation of behaviour is one of the most valuable tools in psychiatry. Therefore, we proposed to adapt the methodology of previous observational studies to systematize emotions identification through facial expressions of psychiatric disorders. Thereby, it would improve the quality of specialized medical training, especially in professionals with extreme values of empathy.
