Abstract:
Nach einem postlingualen Hörverlust kann durch die Versorgung mit Cochlea Implantat das Sprachverständnis bestmöglich wiederhergestellt werden, um die Erhaltung der kommunikativen Fähigkeiten zu gewährleisten. Nach wie vor weist jedoch das Outcome des Sprachverständnisses eine hohe Varianz auf. In der vorliegenden Arbeit wurden maßgebende Faktoren für das Sprachverständnis mit Cochlea Implantat bei postlingual ertaubten Erwachsenen untersucht. Es erfolgte eine systematische Literaturrecherche und eine detaillierte Analyse eines Fallbeispiels eines Patienten nach sequentieller Cochlea Implantation, anhand eines problemzentrierten Interviews und diagnostischer Befunde. Maßgebende Einflussfaktoren bleiben Gegenstand aktueller Forschung. Neue Erkennt-nisse können bereits in die Entscheidungsfindung miteinfließen, die Komplexität einer Vielzahl an Variablen spricht jedoch aus heutiger Sicht in der Praxis gegen Prognosen bezüglich der Sprachverständnis-Performance. Das Fallbeispiel dokumentiert individuell einen erfolgreichen Therapieablauf mit Cochlea Implantat. Tatsächlich entspricht es in mehreren Aspekten der positiven Ausprägung der recherchierten Einflussfaktoren. ; Cochlear implantation enables the rehabilitation of speech comprehension to sustain communication. Nevertheless, the outcome of speech comprehension shows a high variance. This thesis explores decisive factors in speech comprehension with cochlear implants in post-lingual deafened adults. A literature survey and a case study of a patient after sequential cochlear implantation were carried out. Therefor a problem-centered interview and diagnostic findings of the patient were analyzed. There is still an ongoing investigation concerning the decisive factors. New findings can already be included in the decision for or against cochlea implantation. However, from today’s point of view the complexity of multiple variables disables predicting the performance of speech comprehension. The case study individually demonstrates a successful therapy with cochlea implantation. Indeed, several aspects correlate to the positive characteristics of the researched influencing factors.
