Abstract:
This thesis is part of the DECLICS project (Dispositif d’Études CLIniques sur les Corpus Santé) whose purpose is to improve hospital consultations. The health corpus (DECLICS2016) is composed of authentic oral data collected in different services. It includes medical consultations between a specialist doctor and his patient and clinical interviews between a psychoanalyst and a patient. This thesis studies the discourse between caregivers and patients from the point of view of pragmatics. Six medical consultations and six clinical interviews are studied to compare and contrast the speeches of the two types of caregivers (doctors vs. psychoanalysts) with the same patient. The sample covers a total of 10 hours and 26 minutes, for a total of 132,173 words. Five quantitative studies analyze interactional balance, verbal regulation, coherent discourse uses of "mais" and "donc"), local hetero-repetitions and the emotional lexicon. Two qualitative studies examine the interlocutory logic of exchanges in two services, neurology and infectious diseases. Finally, in an application study, via educational formative thumbnails constructed from previous studies, three doctors gathered in a focus group were questioned. The results characterize the first five minutes of discussions, distinguish the roles of caregivers, reveal a variability among psychoanalysts and a homogeneity of the speeches of doctors. The results identify spaces in which the patient knows how to negotiate the learned and experiential knowledge involved in the relationship with their doctor, in order to occupy a discursive place which is useful for the care relationship. The thesis discusses what can be considered as an ideal consultation. ; Cette thèse s’inscrit dans le cadre du projet DECLICS (Dispositif d’Études CLIniques sur les Corpus Santé) qui vise l’amélioration des consultations hospitalières. Des données orales authentiques recueillies dans différents services composent le corpus en santé (DECLICS2016). Il comprend des consultations médicales entre un médecin spécialisé et son patient et des entretiens cliniques entre un psychanalyste et un patient. Cette thèse étudie les discours entre soignants et patients du point de vue de la pragmatique. Six consultations médicales et six entretiens cliniques sont étudiés pour comparer et contraster les discours des deux types de soignants (médecins vs psychanalystes) face à un même patient. L’échantillon porte sur un total de 10 heures et 26 minutes, soit 132.173 mots. Cinq études quantitatives analysent l’équilibre interactionnel, la régulation verbale, la cohérence discursive (emplois de « mais » et « donc »), les hétéro-répétitions locales et le lexique émotionnel. Deux études qualitatives examinent la logique interlocutoire d’échanges dans deux services, neurologie et maladies infectieuses. Enfin, une étude applicative, via des vignettes formatives construites à partir des études précédentes, interroge trois médecins réunis en focus group. Les résultats caractérisent les cinq premières minutes d’échanges, distinguent les rôles des soignants, révèlent une variabilité entre psychanalystes et une homogénéité des discours des médecins. Les résultats circonscrivent des espaces où le patient sait négocier avec son médecin les savoirs savants et expérientiels en jeu pour occuper un place discursive utile à la relation de soin. La thèse discute de ce que l’on peut considérer comme une consultation idéale.
