Abstract:
A ultrassonografi a mamária ganhou destaque na conduta dos mastologistas após o desenvolvimento tecnológico dos aparelhos modernos. Seu uso é difundido principalmente na complementação de mamografias inconclusivas, em mulheres com mamas densas ou com fatores de risco importantes para câncer de mama, porém, não é indicada no uso do rastreamento de câncer de mama em mulheres assintomáticas. Outras indicações que se tornaram importantes são o monitoramento de procedimentos invasivos e a avaliação de nódulos mamários. A ecografia é um método aparelho e examinador dependente e, depois da criação do BI-RADS® ultrassonográfico, diminuíram os índices de variabilidade entre observadores. Publicado em 2003 e adaptado da classificação mamográfica, manteve as características semelhantes, com uma nomenclatura padronizada, classificação de risco de malignidade e orientação da conduta a ser realizada. O léxico BI-RADS® é completo e bem aceito entre os examinadores, entretanto, pelo fato de conter alguns pontos controversos é motivo de crítica de alguns autores. Este artigo se propõe a discutir de maneira crítica a atual situação do uso da ecografia mamária na prática clínica para os mastologia e para os ecografistas, revisando também aspectos técnicos deste exame ; The breast ultrasound has gained prominence in the conduct of mastologists after the development of technological gadgets. Its use is particularly widespread in complementing inconclusive mammograms in women with dense breasts or with signifi cant risk factors for breast cancer, however, is not indicated in the use of breast cancer screening in asymptomatic women. Other important indications are the monitoring of invasive procedures and evaluation of invasive breast nodules. Ultrasound is a method examiner and device dependent, however, after the creation of sonographic BI-RADS® has decreased the levels of variability among observers. The sonographic BI-RADS® was published in 2003, adapted from the mammographic BI-RADS® itself, maintaining similar characteristics, with a standardized nomenclature and malignancy risk classifi cation and guiding the conduct to be performed. The BI-RADS ® lexical is complete and well accepted among the examiners, however, because it contains some controversial points is a matter of critic of some authors. This article proposes to discuss critically the current situation of breast ultrasound use in the clinical practice of mastologists and ultrasonographers, also reviewing the technical aspects of this examination
